I totally agree with Mona Othman here. This painting is a personal expression of the artist's belief, that's why she drew the American flag as a badge of honor being hung on his chest. Not only that, but she believes in him to the extent that she didnt make the background the American flag like the original photo, she mixed the flag with his face/head to symbolize that he's always thinking about his country & maybe she did that to symbolize that he's his country BEFORE himself
